- Completed application
- Psychological and/or educational testing
- Most recent physical exam
- High school transcripts
- Front and back of insurance card

In addition to touring the Vista campus, you will also meet the key people that will be working with your son, including his primary therapist, the school principal, a recreation therapist, a substance abuse counselor (if applicable), and most importantly, some of the currently enrolled students. To schedule a tour please contact the Admissions Director, Raymond Archuleta at 801.558.3335.
Immediately following your decision to enroll your son at Vista, the admissions team will begin helping you make plans for a smooth transition. Vista will provide you with an electronic copy of the admission application, as well as a packing list. From there it's just a matter of coordinating the day and time of arrival.
The cost is $400 per day.
